If you’ve had any X-rays, CT scans, or MRIs performed related to your problem, please be sure to bring them with you to your appointment. Please also bring a list of medications that you take on a regular basis. If you need to cancel or reschedule your appointment, please be sure to provide a 24 hour notice to the office in order to avoid a cancellation fee. Due to the amount of time blocked for VNG testing we require a 48 hour notice for cancellation of a VNG test in order to avoid a fee.
If your insurance carrier requires a referral for specialist services please obtain one prior to your appointment. If a referral is not obtained by you prior to your appointment you may be asked to reschedule your visit.
